Recent trials and outcomes indicate that carotid artery stenting (CAS) is an alternative to carotid endarterectomy (CEA) for high-surgical risk patients. Even as the CAS vs. CEA debate continues, the crucial question is how do we achieve successful patient outcomes with carotid revascularization? Can the results of the FDA pre-market approval trials, largely performed in large centers with high volumes be translated into community hospitals? When is CAS a better option than CEA? Should we be stratifying high surgical risk patients by anatomic features or physiologic comorbidity, in order to optimize patient selection for CAS?
Two leading authorities will share their real world outcomes in an exclusive Webcast on May 6th. Join us for this complimentary presentation featuring Dr. Douglas W. Massop of The Iowa Clinic, and Dr. William A. Gray of Columbia University.
Dr. Massop, a vascular surgeon, will share trial data from the SAPPHIRE WW trial (Stenting and Angioplasty with Protection in Patients at High Risk for Endarterectomy World Wide). SAPPHIRE WW is the post-market surveillance trial based upon the original SAPPHIRE trial, which was the only randomized comparison of CAS and CEA in high-surgical risk patients which demonstrated early and late non-inferiority by intention to treat, and superiority by treatment received for CAS compared to CEA in high-surgical risk patients.
Dr. Gray will discuss findings from his recently published “30-Day Outcomes for Carotid Artery Stenting in 6,320 Patients From Two Prospective, Multicenter, High Surgical Risk Registries” that appeared in March 2009 Circulation: Cardiovascular Interventions. He will address the issue of CAS achieving the AHA benchmark outcomes, originally established for carotid surgery, of 30-day outcomes for both asymptomatic and symptomatic patients at increased surgical risk for CEA.
Facilitating the discussion will be two leaders in carotid artery stenting, Dr. Christopher White of Ochsner Clinical Foundation, and Dr. Kenneth Rosenfield of Massachusetts General Hospital.

CMS Move to Revisit Decision on Expanded Coverage for Carotid Artery Stenting (posted: 04/08/09)
Please see The Society for Cardiovascular Angiography and Interventions's (SCAI) comments pertaining to CMS revisiting its decision on expanded coverage for carotid artery stenting (CAS)
Expanded coverage of CAS to include all Food and Drug Administration (FDA)–approved indications would provide additional treatment options for a larger group of patients who are at risk for stroke. This would reverse the agency’s Oct. 16, 2008, decision not to expand coverage of CAS at that time, which cited a lack of published, peer-reviewed data.
