A Longitudinal View of Patient Care

The Diabetes Collaborative Registry provides a unique, longitudinal view of the presentation, progression, management, and outcomes of patients with diabetes as they receive care and treatment from multidisciplinary care teams.

Leveraging the well-established technology platform of the ACC’s PINNACLE Registry®, the nation’s largest ambulatory cardiovascular registry, the Diabetes Collaborative Registry builds upon the growing number of patients with diabetes in the PINNACLE Registry and retrieves additional clinical data from electronic health records across a wide spectrum of participating providers. Improved Patient Outcomes

The Diabetes Collaborative Registry draws measures predominantly from existing Physician Quality Reporting System (PQRS) measures and retrieves data from electronic health records to facilitate coordinated care, prevention and treatment of diabetes.

Reports

The Diabetes Collaborative Registry will provide easy-to-interpret benchmark reports that validate the quality care you provide and pinpoint opportunities for improvement.
