Clinical Data Registries

Veradigm offers two clinical data registries operated in association with the American College of Cardiology (ACC). The registries provide a longitudinal view of the presentation, progression, management, and outcomes of patients with cardiovascular disease and diabetes as they receive care and treatment from multidisciplinary care teams.

Veradigm Cardiology Registry (formerly PINNACLE Registry) is the largest outpatient quality improvement registry, capturing data on coronary artery disease, hypertension, heart failure and atrial fibrillation.

Veradigm Metabolic Registry (formerly Diabetes Collaborative Registry) is the first clinical ambulatory registry aimed at tracking and improving the quality of diabetes and cardiometabolic care across the primary and specialty care continuum. This interdisciplinary effort was developed by the American College of Cardiology in partnership with the American Diabetes Association, the American College of Physicians, the American Association of Clinical Endocrinologists, and the Joslin Diabetes Center.

Both registries are currently enrolling practices across multiple specialties, specifically primary care, family care, internal medicine, endocrinology, and cardiology.

Veradigm has partnered with the American College of Cardiology (ACC) to advance the next generation of real-world research on behalf of cardiovascular disease and diabetes patients around the world. As part of this partnership, Veradigm operates the Veradigm Cardiology Registry and the Veradigm Metabolic Registry.

Benefits of Registry Participation

The data included in the Veradigm Cardiology Registry and the Veradigm Metabolic Registry are collected from cardiologists, primary care physicians, endocrinologists, and other providers to provide an overview of patients with cardiovascular and/or cardiometabolic disease. This data is a valuable resource for longitudinal studies which can reveal new insights, enable better treatment, and improve outcomes.

Registry participants can access easy-to-interpret benchmark reports to facilitate coordinated care, prevention and treatment of diabetes and cardiovascular diseases. By drilling down to the patient level metrics, clinicians can easily identify opportunities for improvement.

Participation in each of the registries has many advantages including:

  • The ability to gain deeper clinical insight not currently available to you on patients with:
    • Coronary artery disease, hypertension, heart failure, and atrial fibrillation
    • Diabetes type I and type II, prediabetes, and cardiometabolic care
  • Free MIPS reporting and quality measures for improvement in patient care
  • Easy-to-interpret online benchmark reports
    • Help you validate the quality of care your practice provides and pinpoint opportunities for improvement
  • Seamless participation in federal quality reporting programs, including the Quality Payment Program:
    • Eligible clinicians and practices can earn five bonus points for Clinical Data Registry Reporting (PI_PHCDRR_5) for the Promoting Interoperability performance category by participating in the Veradigm Cardiology Registry and/or the Veradigm Metabolic Registry.

As part of our partnership with the American College of Cardiology (ACC) participants also have:

  • Access to the ACC’s NCDR suite of clinical toolkits to help implement best practice care and address common quality issues
  • Opportunity to submit hypothesis-driven research requests based on analysis of ACC’s NCDR suite of data registries
  • Ability to participate in continuing education programs sponsored by the ACC

Reporting

The Veradigm Cardiology Registry and the Veradigm Metabolic Registry “Practice Executive Summary Quality Reports” allow practices to validate the quality of care provided, benchmark performance on a national and practice level, and identify opportunities to close gaps in care. To generate these reports, data for all relevant patient encounters must be collected and submitted via the data collection process.

Data Collection

Submitting data to the Veradigm Clinical Data Registries is seamless, secure, and HIPAA compliant. . Data collection for the Veradigm Cardiology Registry and the Veradigm Metabolic Registry is managed via an arrangement with our technology partner FIGmd. Relevant registry data elements are captured through secure HIPAA-compliant data collection methods.

The data that are collected through the Veradigm Clinical Data Registries are de-identified before they can be licensed by any third party.

DATA PULL PROCESS

System Integration (SI) where data is shared securely via easy to install software that works with most electronic health record (EHR) systems.

SI is compatible with over 80 EHR and Practice Management Systems including Veradigm/Allscripts, Cerner, GE Centricity, GEMMS, NextGen, Greenway, Intergy/Sage/Vitera, Amazing Charts, Soapware, and eMDS.

DATA PUSH PROCESS

Secure File Transfer Protocol (SFTP) where the required data elements are exported by the practice to a data file supplied to our technology partner (FIGmd).

  • Provider and facility characteristics
  • Other indicators examining provider adherence to guideline directed non-statin therapies
