Diabetes is a multifaceted disease that causes a range of complications for patients and has a myriad of different treatment and lifestyle options for managing those conditions. The capability for patients and health care providers to have a resource to reference and track patients’ health experiences can be an invaluable resource.

To help practices engage with their patients, the Diabetes Collaborative Registry has partnered with PatientsLikeMe, an online patient network and real-time research platform where people living with chronic illnesses can find: 

  • Peer-to-peer support
  • Track their conditions
  • Record health data that fully reflects their real-world experiences

Over 400,000 patients representing more than 2,500 conditions are already using PatientsLikeMe to connect in forums and to track and share their experiences, including reporting the severity of symptoms and the effectiveness of treatments

How can your practice participate?

As a PatientsLikeMe participant, patients will have access to the following tools and resources:

  • Tracking: Patients can track their health with easy-to-use tools that show what’s changing over time. Then, they can take the facts they collect to their next visit to your practice for a more productive conversation.
  • Connecting: Patients can connect with others who know what they are going through. Members voice their experiences and feelings about thousands of different conditions.
