We collect and use non-personally identifiable information for the purposes of quality control and improving the NCDR™ Site.
The "help" portion of the toolbar on most browsers will tell you how to prevent your browser from accepting new cookies, how to have the browser notify you when you receive a new cookie, or how to disable cookies altogether.
If you choose to disable cookies altogether, you will not be able to login to the NCDR™ site with a personal User Name and Password. This will prevent you from being able to make use of the advanced personalization features of the NCDR™ Web Site, although the basic functionality offered to unregistered users will be possible.
Granting Access to the NCDR™ Site
In order to access certain content on the NCDR™ Site, the institution must be a registered participant of the NCDR™. Once a Participant Agreement has been executed and participation fees have been received, the institution will be required to assign a Registry Site Manager. The Registry Site Manager will be assigned a personal User Name and Password by the ACCF for the purpose of administering this Web Site. This person assumes the responsibility for setting up additional User Names and Passwords for the institution. The User Names and contact information for all users established by the Registry Site Manager will only be accessible by the institution and the ACC. This information will not be made available to any 3rd parties. The contact information of the Registry Site Manager as well as institution demographic information may be made available to other NCDR™ participants if the Registry Site Manager elects to do so. The Registry Site Manager may go to the "Site Profile” link for information on how to opt-in or opt-out of use of this information. The information that will be made available to other NCDR™ participants include but are not limited to the Registry Site Manager’s name, e-mail address, business phone number, hospital name, city, state, zip and cath lab volume.
The User Names and Passwords assigned to the NCDR™ site users should be strictly controlled and should not be distributed to any persons outside the Institution. Once the Participant Agreement has terminated, all User Names and Passwords for the Institution will be automatically disabled.
Access to your User and Site Profile Information
The information maintained for each user assigned a User Name and Password can be reviewed and modified at any time only by the validated user by clicking on the "User Profile" link. The accuracy of the user data is solely the responsibility of the user.
The information maintained by the Registry Site Manager about the institution can be reviewed and modified at any time by clicking on the "Site Profile" link. The accuracy of the user data is solely the responsibility of the user.
All authenticated users of the NCDR™ web site may automatically receive e-mails relevant to the functionality of the NCDR™.
Retention of Data
Data collected from you will be retained as long as the information is relevant to our business purposes.
Our Commitment to Data Security
The foregoing policies, as well as the Privacy terms set forth in the Participant Agreement and incorporated herein, are the current Privacy Policies of the NCDR™ site. If we change this policy, we will put a notice on the Home page of the NCDR™ site. Our statement and policies do not create a contract or any legal rights.
When you visit the NCDR™ site you could be directed to other sites. These sites are beyond our control. You should be aware that other Web sites to which you link may contain privacy provisions which differ from the privacy policies of NCDR™ site.
