The NCDR™ Web Site is owned and operated by the American College of Cardiology Foundation ("ACCF"). The Participant Agreement executed between the ACCF and your Institution, permits the ACCF to grant your institution access to this Web Site. The information collected by this Web Site for all users accessing the site is subject to the terms of this Privacy Policy.

 Collection of Non-Personally Identifiable Information Through Use of Cookies

We cannot determine your personal identity unless you are a registered participant on the NCDR™. You may use certain features of the NCDR™ Web Site, such as browse, as a surfer without being a participant. In this case, we do not collect any personally identifiable information about you, but we do collect and use non-personally identifiable information regarding your use of the NCDR™ Site through the use of cookies. Cookies are anonymous alphanumeric identifiers stored by us on your computer which enable us to identify your computer when you access the NCDR™ Site.

Examples of non-personally identifiable information we collect and analyze from all users, registered or unregistered, include the Internet protocol (IP) address used to connect your computer to the Internet; computer and connection information such as browser type and version, operating system, and platform; click stream data, including date and time, cookie number and content you viewed or searched for

We collect and use non-personally identifiable information for the purposes of quality control and improving the NCDR™ Site.

The "help" portion of the toolbar on most browsers will tell you how to prevent your browser from accepting new cookies, how to have the browser notify you when you receive a new cookie, or how to disable cookies altogether.

If you choose to disable cookies altogether, you will not be able to login to the NCDR™ site with a personal User Name and Password. This will prevent you from being able to make use of the advanced personalization features of the NCDR™ Web Site, although the basic functionality offered to unregistered users will be possible.

We will not disclose personally identifiable information obtained by the use of cookies to third parties.

 Granting Access to the NCDR™ Site

In order to access certain content on the NCDR™ Site, the institution must be a registered participant of the NCDR™. Once a Participant Agreement has been executed and participation fees have been received, the institution will be required to assign a Registry Site Manager. The Registry Site Manager will be assigned a personal User Name and Password by the ACCF for the purpose of administering this Web Site. This person assumes the responsibility for setting up additional User Names and Passwords for the institution. The User Names and contact information for all users established by the Registry Site Manager will only be accessible by the institution and the ACC. This information will not be made available to any 3rd parties. The contact information of the Registry Site Manager as well as institution demographic information may be made available to other NCDR™ participants if the Registry Site Manager elects to do so. The Registry Site Manager may go to the "Site Profile” link for information on how to opt-in or opt-out of use of this information. The information that will be made available to other NCDR™ participants include but are not limited to the Registry Site Manager’s name, e-mail address, business phone number, hospital name, city, state, zip and cath lab volume.

The User Names and Passwords assigned to the NCDR™ site users should be strictly controlled and should not be distributed to any persons outside the Institution. Once the Participant Agreement has terminated, all User Names and Passwords for the Institution will be automatically disabled.

 Access to your User and Site Profile Information

The information maintained for each user assigned a User Name and Password can be reviewed and modified at any time only by the validated user by clicking on the "User Profile" link. The accuracy of the user data is solely the responsibility of the user.

The information maintained by the Registry Site Manager about the institution can be reviewed and modified at any time by clicking on the "Site Profile" link. The accuracy of the user data is solely the responsibility of the user.

 E-mail Communication

All authenticated users of the NCDR™ web site may automatically receive e-mails relevant to the functionality of the NCDR™.

 Retention of Data

Data collected from you will be retained as long as the information is relevant to our business purposes.

 Third-Party Advertisers

We contract with third-party advertisers who post banner ads on the NCDR™ site, which may link to other Web sites not under our control. These third-party advertisers may use cookie technology to measure the effectiveness of their ads or may otherwise collect personally identifiable information from you when you leave this site. We are not responsible or liable for any content, advertising, products or other materials offered from such advertisers. Transactions that occur between you and the third party are strictly between you and the third party and are not our responsibility. You should review the privacy policy of any third-party advertiser, as their policies may differ from ours.

 Our Commitment to Data Security

We recognize that your privacy is important to you, and therefore we endeavor to keep your information confidential. ACCF incorporates Section 4 of the Participant Agreement into this Privacy Policy. Institution information, including but not limited to Outcomes reports, Data Quality reports, User Profile and Site Profile are linked directly to an NCDR™ User Name and Password and can only be retrieved by supplying the correct User Name and Password. The personal and demographic information about each user is stored on NCDR™ servers that are behind a firewall and physically housed within a secured data center. Furthermore, our internal practices help protect your privacy by limiting employee access to and use of customer data. However, we do not represent, warrant, or guarantee that your institution information will be protected against unauthorized access, loss, misuse, or alterations, and do not accept any liability for the security of this information.

 About This Privacy Policy

The foregoing policies, as well as the Privacy terms set forth in the Participant Agreement and incorporated herein, are the current Privacy Policies of the NCDR™ site. If we change this policy, we will put a notice on the Home page of the NCDR™ site. Our statement and policies do not create a contract or any legal rights.

This privacy policy applies solely to the information you provide while visiting this Web site. The terms of this privacy policy do not govern personal information furnished through any means other than this Web site (such as telephone or via the mail).

When you visit the NCDR™ site you could be directed to other sites. These sites are beyond our control. You should be aware that other Web sites to which you link may contain privacy provisions which differ from the privacy policies of NCDR™ site.
